As salamu Alaykum A relative is deeply in debt (due to financial and health reasons). They cannot currently afford to pay off their debt. If I give them enough money to pay off their debt , where the amount is even more than they need for 1 year of sustenance, will this all be considered ZAKAT?

Yes, if he is not able to pay it off and provide then u help him with the zakat
