It depends on her capability. If she is not able to do it herself, then perhaps a nurse or someone she is comfortable with can change the diaper for her before every salah. What she needs to do is: 1. Purify herself 2. Make wudu After that, whatever happens is not a problem; she can pray without having to pay attention to it. Of course, she does this for every salah.
